Prior Authorization Update for HCPCS 0345U: Effective October 1, 2026

Effective October 1, 2026, management of HCPCS code 0345U (GeneSight^® Psychotropic test) will transition from Wellcare partner EviCore to WellCare of Kentucky. Prior authorization and utilization management will be administered by the Health Plan using internal utilization management processes.

Providers should submit authorization requests for HCPCS code 0345U directly to the health plan in accordance with established prior authorization requirements and submission guidelines. Additional prior authorization information is available on the WellCare Provider Authorizations site.
